What to Bring to the Office:

Please bring the following things to the office.  This will help to expedite the check-in process:

• Picture ID (Drivers License, State ID Card, or Passport

• Medical Insurance card

• List of medications you are currently taking

• List of any changes to your general health

• List of questions you have for the Doctor

• Forms (if you downloaded them online and filled them out prior to the appointment)
